It looks like it is diseased. What is going on? Going to try some NEW silver nitrate??? Hopefully that is it?

What is this? I can't figure out what is going on? I think it has to be the silver nitrate but I filtered it 2xs. Checked the ph & the hydrometer reading was the same???
